I forgot my username and password?

Select your organization and your username and password will be sent to the email address of the person that created the listing.

I can't go "Back"!

The system walks you through a step-by-step process for registering your organization. Your information is saved as you go through each page so you can stop at any time and return to where you left off. For this reason, when you fill out the form the first time, you cannot go back to edit previous pages. If you need to change something, complete the registration process first. Then, you will be able to return to and edit any part of your listing.

I couldn't complete my registration, what do I do now?

If you do not have all the information required to complete your registration, you can stop at any time and return to the registration process. When you have collected all of the necessary information, return to the site, go to the By Name view, open your listing, click the "Edit" button and re-enter the username and password originally sent to you by the CHPCA. After completing the registration your private username and password will be sent to you allowing you to make future updates to your listing.


I encountered an error or problem filling out the registration.

Click on "Comments" on the left and explain the difficulty you encountered. Please explain what you were doing at the time and what type of error or problem you encountered. Someone will contact you and help you complete the registration process.

I did not receive my confirmation email

Find your organization's name in the By Name view. Click on "Edit" and re-enter the username and password sent to you by the CHPCA. If the username and password are accepted, you will be returned to the last page of the questionnaire. Make sure you fill in a valid email address, then click the Finish button. You should then receive your confirmation.

If the original username and password are not accepted, enter the new username and password you selected at the beginning of the process. If you are allowed to proceed, write down the username and password and store this in a safe place. Check the email address you entered on Page 8 to ensure it is valid.
